What is on a US military dog tag... I am looking at one right now... LAST NAME, FIRST NAME MI. SSN BRANCHOFSERVICE BLOODTYPE RELIGIOUS AFFILIATION Example: SMITH, (could have JR or SR or other designation after the comma) JOHN J. 123456789USN A NEG (no space between SSN and Branch)(blank) RC (the religious designation is in the bottom left corner) The above format is the current US NAVY, they have different formats depending on the era and branch of service.
