1)what is the common incident happened on board ship?

2)what makes it happen?

3)what are the action to prevent it?

This is too broad a question to answer. Are you referring to personal accidents? If you are referring to incidents, maybe big waves and strong winds, but precautions are taken to tie up all movable objects whenever there is an approaching bad weather.

What I meant is bad incident that usually happen to the ship's crew. Incidents like fire or man overboard, but I'm looking for the common one. I'm not sure if it is common or not.

Dear Mohd Iskandar,

From what I see, there will be more incidents like these as the ship ages.(comparing new ships with old ships) The quality and competency of the crew manning the ship and the support from the shipping company with good safety practices also plays an important part to prevent accidents.

As you have mentioned, fire, man overboard, blackout, machinery breakdown, oil spill, falling, injury from hand tools, etc. You name it, it can happen. Unfortunately I am not able to tell you what usually happen because all these are well known and can be prevented if the above-mentioned crew and management are vigilant and competent in what they do.

For example man overboard seldom(or never) happen because the crew know that they will not go out on deck when the weather is rough.
