Intro to logic question (philosophy). Where did I go wrong?

I'm supposed to translate the following argument into symbolic form, using the letters provided in the order in which they are listed and then complete a proof for this argument, but I can't finish the proof, so I'm assuming I just translated it wrong... but where did I go wrong?

If either medical fees or malpractice awards escape restrictions, then health care costs will soar and millions of poor will go uninsured. If the lawyers get their way, then malpractice awards will escape restrictions. If the doctors get their way, then medical fees will escape restrictions. Either the doctors or the lawyers will get their way, and insurance companies will restrict reform. Therefore, health care costs will soar. FAHPLDI

4. (D v L) + I Deduce: H

Your symbolization is correct.

Edit: CD is Constructive Dilemma and MP is Modus Ponens.
