All the Insurance policy might not be same. For the most part, Insurance companies set malpractice premium according to Physician's specialty, type of practice and geographical locations. Typical malpractice insurance policy covers any loss up to certain amount for an individual case or losses within a year up to some amount. Fir more detail you can check the following source.

Um yea, fair's fair. What's so unfair about insuring a doctor in case they get sued?
