Collision report to dmv?

Hi,

couple of weeks back i hit a rock from side, which is right on the curt at one of the costco parking lots. My insurance company sent me a dmv form to report it. Do i need to report it? There is no property damage.. in fact my car is damaged.

Yes. You must report it. Property damage means ANY damage (including yours) that occurred.
