Family Law - Colorado - Father needs help?

I have a few family law questions I have for the state of Colorado. I don't have $5,000 to $7,000 to pay to a lawyer and I would like to just fill out the forms and do this on my own. But I don't know what forms I should fill out.
Here is my story:
My high school gf got pregnant when she was 16 (i was 18) and moved out of state (Ohio to Colorado) w/o telling me she was pregnant. When she gave birth she gave up her rights to her mom who raised my daughter. When my daughter turned 9 years they told me about her. I moved from Ohio to colorado to do a dna test and be apart of her life. The dna test came back that I was her dad. But shortly after the family took off with my daughter and are hiding her. I want to be in her life. I would like to see her on weekends and even asked the grandparents to let her go to her little sister's 1st birthday and they blocked me on facebook. I went to the mother's work and she refused to give me her mother's number so I could talk to her.
So this is where I am...I need help but unsure where to go from here. Thank you for any help or advise.
