Dear Brand,

By law, you MUST amend your tax return to report the missing income---and, perhaps, the withholding that will help mitigate the resulting increase in tax liability.

The correct federal form to use is Form 1040X.  Here's the URL from the IRS' Web site for this form:http://www.irs.gov/pub/irs-pdf/f1040x.pdf

Here's the URL for the instructions to Form 1040X:http://www.irs.gov/pub/irs-pdf/i1040x.pdf

Brand, I hope the above is helpful.

Sincerely,

James Rogers CPA
