How do I get a certificate of good standing from the United States Patent & Trademark Office (USPTO)?

I am a Registered Patent Agent. I am applying for the PA Bar Exam and I need to provide a Certificate of Good Standing from the Patent Office with my bar application. I know that they send them out because they list the fee for doing so in their rules as $10. However, I cannot find the procedure for doing so *anywhere*. Please help!

I'm not sure of the exact procedure. But, I assume the Office of Enrollment and Discipline would be the office that issues them. Therefore, I assume that a call to that office would answer your question.....Of course, I would make sure that the procedure isn't clearly spelled out somewhere in 37 CFR...
