I have a Niece who is Acting Out emotionally?

I have a niece who is 11 years old. She was raised by my Mother and Step Father. My Sister was taking care of her then her and the father broke up and my sister thought it best to give my Mother custody of her daughter. My Sister and her boyfriend got back together and they got married. They had two more children. My Mother kept my niece. For a few years my Sister never contacted my family because of the Husband's Mother having control over them. The Mother In Law died...My sister started being friendly with us again. My Niece would go over there and spend the night. My niece complains about her sister who she can't stand who is younger then her. She loves the youngest girl. Anyhow, she drew a photograph of her dead and the middle sister laughing that she died. Then she started crying saying that she doesn't feel loved. She don't feel that her Mother loves her. My sister claims she needs to seek professional help.....Something isn't right. Any thoughts? Thanks
