Name Change Back Again To Original

Although sworn in being an American resident I altered my authorized title into anything completely different in storage of my precious sibling who died a sad demise before he turned a health care provider. And so I possess the new title on my citizenship document and passport. But about the back site of the National passport I also pay "AKA my unique delivery title". But I've never utilized this new title on every other files like driver's permit, tax types, SS card and etc. Today I do want to utilize my unique delivery title about the front-page of my passport, what do I've to complete? Thank you.

To legally change your name you go to court and ask for it to be done.
You can, however, use any name you like so long as it is not done for fraudulent purposes.
