I typically assmed it had been an 'American' branch of BP -- but British Petroleum is as British as the Royal Dutch "Shell" Petroleum is Dutch. Proper? It had been a British organization, not American, Consequently who did this?

All of the seven sisters(the big oil companies) are True multi nationals(Yes the B did originally stand for British.) Yes there is are several US divisions most separate legal entities. Royal Dutch Shell is Anglo-Dutch BTY. Legally a company has to be registered some where but often this is for Tax purposes for example Camen Islands or Switzerland.So it was a multinational or US company that did "this".

So, yeah, the ultimate fault is with President Obama's appointee who did not do her job of ensuring that the oil well had the right equipment operating the right way. Some have questioned whether the President's appointee even knew what these right ways would've been.
