Reccomendation on a Workmans Compensation form?

The Workmans Compensation case worker has tried to pressure me into signing this form.. Can you please give me your opinions on what you think I should do? Why would the case worker want me to sign this? It is Workers Compensation Insurance.


"I, [Recipient] have been offered and accepted Job Placement Assistance (JPA) in the amount of $7500.00 The first installment was made on 03/30/09 in the amount of $3,750.00. The second installment was hand delivered on 08/28/09 in the amount of $3,750.00 I understand that these funds should be allocated to assist me in my search for employment. Furthermore I was offered and accepted an advance of $2000.00 which will be credited from any future settlement agreement we may reach. I understand I must achieve maximum medical improvement (MMI) from both treating physicians before any settlement thoughts can be entertained."


Please note that the second deposit of $3,750.00 was never given to me. Thank you for any comments or suggestions.

if you did not receive the funds don't sign
if you did receive the funds there is no reason not to sign.
all you are signing is a written record of what has transpired to this point..

you must reach MMI before a settlement can be offered due to the fact that a rating will be determined based on your permanent disability.
after MMI is reached your doctors will determine your percentage of permanent disability. that percentage along with your age, occupation, education, previous work history etcetera are all used to determine the settlement you are paid. this is all set by the state and the state must approve the settlement.

if you have any concerns you should contact your states Workers Compensation Commission they are there to assist you and to ensure you are treated fairly and according to all the laws of your state.

this link will take you to the US Department of Labors site that list every states WCC if you do not know how to contact them already.

I would never sign that without an attorney looking at it. The topic guy is trained to put the company intrest over your interest. $ 7, 500? Really...

They already breeched the deal...
