The third choice is correct: A lawsuit filed against a lawyer for serious error that results in injury or loss.
The first just sucks for aspiring lawyers who just paid an arm and a leg for their legal education, but they can always retake it.
The second is just not acting ethical by the state bar's professional model rules of ethics and professional responsibility standards. The consequences are that a complaint might be issued and the lawyer could be censored, fined, or even lose their bar license, or all or a combination of those three things.
The third is legal malpractice and you have to prove that, not only the lawyer was negligent, but that you would have won your case, if they hadn't screwed up so bad.
The fourth sort of ties in with the second but it is more subjective.
