I've been attempting to study this subject before I move foreward with altering my boy's lastname (ideally) but I would like to request a couple of questions so I've a much better idea what Iam facing.
Our son is a couple of years old, he's his dad's final name.  Our ex and I were never-married, we divided when my son was a few months old since he was violent/verbally abusive.  Initially we'd shared custody and I'd major physical custody... he started with supervised visitation at my parents house, which eventually resulted in supervised visitation by his (my son's dad) parents.  In Nov. 2010 once I selected my son up from one of the visitations, my ex turned bodily w/ my son there and was imprisoned for many costs, one including threat of damage to some minor.  Until now he'd been mainly present with child support, without the arrearage.  After his charge, the child support stopped coming in.  The domestic abuse plan granted a restraining order on him for me.  Since that time - he was permitted to have supervisied visitation again w/ my parents, which he started initially to hit off & stopped not having notice.  He'd stopped paying child support & when my attorney suggested he start paying, he decided he desired to sign his rights over.  The judge advised him that though he would not be paying child support during this period (awaiting the reading to sign his rights around), child support might still stand up incase he decided to alter his thoughts - which he did.  Iam sorry this is really long - ever since then I've obtained Single custody both bodily and primary.  He Hasn't followed through with getting parenting classes or calling to setup supervised trips by a company like our court documents said.  He's not often paid his child-support and owes more than 6,500.  We currently had your final contract w/ the court and now Iam thinking can I now move ahead to alter my boy's lastname?  I understand I've to prove he's inconsistant, which you can clearly see he is.  From what you can see there, what kind of documents might prove the greatest? I'd prefer to move ahead with my entire life and have my boy have my last name.  Thanks so much in advance.  I reside in Connecticut.

Based on the CT courtroom site you document the application form after which the judge includes a reading. Browse the methods and also the types at:
http://www.jud.ct.gov/lawlib/Law/namechange.htm
Provide every shred of paperwork as you are able to gather. You'll discover quickly enough in the reading if you want another thing.
May be advisable to consult a lawyer that has expertise having a minor's name-change and obtain some concept of exactly what the judge searches for.
