Filing for A Change In Custody. Can someone help?

I live in NC and my two girls ages 12 and 6 are living with a horrible excuse for a mother who has actually been reported to Child Protective Services twice, does drugs, among other things. I will soon be filing for motion for a change in custody and I need help. I have a long list of things that I need to put in the modification of custody paperwork "reasons" section but I need help in writing them in a professional manner. In my state we have to go to mediation before we actually go to court so its cheaper for me to do this part myself instead of paying a lawyer twice. If someone who has a background in Law could help me re-word my "reasons in a professional manner I would be very grateful. Our legal aid office does not help in Family Law and we do not have a self help center. Please send me an email address. Thanks so much.
