I need help with the rules of passive voice.?

Hello, I need some help turning sentences from passive voice to active voice. Here are the sentences I need to change:
The president of Youth Research Inc was sentenced to prison and fined for her role in the conspiracy.
Research was found guilty and sentenced to two years probation and ordered to pay fines for committing conspiracy.
He found that the same children participated in repeated tests. He also found that reported birthdates, genders, and schools were inconsistent.
The health scientist found misspelled names, and forged parental signatures.
The case was referred to U.S. Department of Justice’s Office of Consumer Protection Litigation.
It was determined that Youth Research and its contractors submitted fraudulent data in connection with almost every test performed
