Where can i find a work deferrment form?

i am 16, and want to work more hours then child labor laws will allow. i was told there is a work deferrment form that i just need a parent signature to be able to work more. does anyone know where i can get the form? by the way, i live in michigan, if the forms vary state to state.

Call your local Department of Labor Ask about child labor laws for your state. Good Luck
