How Do I Remove A False Account Of Debt Collections On My Credit Report?

I recently obtained my credit report and found there is a collections account for a credit card company I never had on it. This account says that is was opened for collections in Jan 2013. I never had this credit account, and this account does not appear on my credit reports for 2011 or 2012 from the original creditor. I paid off all my 10+ years old cards. I know this is false information, so how do I remove it? Its the only negative mark and its effecting my score!
