I would let every adult to deal with his health problems the way he wishes. It's not government's job to deal with them. Government would only take care of children and very few adults who genuinely can't work (and this inability to work is not due to their own faults). Perhaps an individual mandate covering strictly catastrophic health problems in order to cover costs for emergency care which otherwise would have to be paid by the rest of the people. This would cover only health problems that can lead to immediate death, eg ruptutre of an aortic aneurysm.
