What Is The Worst Health Insurance Company You Have Dealth With And Why?

I have had a horrible time with the insurance company my employer just switched to. I'd genuinely like to know what you all go by way of with your insurance businesses.

HealthPartners Technically, the problem was dealing with them after I was no longer a customer(transferring from my parental units' insurance to my employers insurance). They wouldn't pass my medical records to my new doctor, unless I paid $1.50/page plus more for the mailing, because I was no longer a member. After negotiating a bit, I just gave up and had my new insurance cover a fresh physical. I guess I feel like my medical records should be "mine"; that is, the insurance company or my doctor or whatever are just holding them for me, in their systems, but I should be able to move them, copy them, destroy them, whatever. Oh, also - I had Delta Dental with my parental units, and Delta Dental with my employer, but on graduation day I entered some kind of undefined dental state where I couldn't get my 6-month checkup, even though I had been hired.

I made the horrible mistake of paying for an in-network visit.  I had just changed to Guardian, and my last insurer required pre-payment and then a claim submittal.    It took 18 months with increasingly strident emails and calls before I was able to get the money refunded back to the doctor, and then another 3 months to convince the doctor's accounting department to send that money back to me!   I kept getting different advice from Guardian each time I called and at one stage they tried denying my claim as being out of time after they had told me to wait 2 weeks before checking on the claim again!   And when my son broke his jaw and had his mouth wired shut for 6 weeks, they declined to cover for him to get a cleaning at the dentist after the wires were removed, as it was less than 6 months since his regular cleaning!
